Insurance/payment information

Through a partnership with Lactation Consultants of America, we accept the following insurances:

• Aetna/Meritain
• Anthem
• Blue Cross Blue Shield
• Cigna
• Health Net
• Oscar
• United Healthcare/UMR/GEHA/Empire/Oxford

To verify your insurance is accepted, please complete the intake forms provided after scheduling an appointment. Once submitted, we will verify your insurance benefits.

If you are self-pay, you will be provided with a superbill, which is an itemized form, that you can submit to your insurance for possible reimbursement. Superbills for lactation support are not guaranteed to be paid or paid in full by insurance. Lactation consultations are eligible expenses if you have Health Savings Account or Flexible Savings Account (HSA/FSA). Payment must be paid in full at the time of the visit. Cash, Card, and Check are accepted. Prices listed for each service in appointment scheduling.
